Support Coordination
Support coordination is crucial for finding and coordinating the necessary services, resources and supports within the community that are required by adults and children with intellectual and/or developmental disabilities and their families.

Plan of Action
Individuals with intellectual and/or developmental disabilities and their families often have a hard time finding and coordinating the services, resources and supports they need to ensure a high quality of life and full inclusion in the community. Service systems can be challenging, complex to navigate and are often critically underfunded. Determining funding sources for necessary services can be extremely difficult. US Home Care’s support coordinators will help you in discovering the potential of your loved ones and the ways supports and services can be utilized. We assist you in developing your personalized plan and in coordinating and monitoring your plan and services.
Personalized Plan
First, during a series of initial meetings and phone calls, Heart To Heart support coordination will assist you in utilizing and coordinating with community resources and other programs to develop your plan. Supports of family, neighbors, and community will be explored. Programs and services by local, state, and federal agencies will be examined. Our support coordinators will provide help and coordination with all appropriate supports. This is how we develop your one-of-a-kind service plan that will become the blueprint for the future.
